IEP tracking
I created a form that I could use to track student IEP goals. This would be a great way to get teacher input on how a student is doing in mainstream classes in a particular subject or even on overall social skills. I also saw an area where you can get a summary of the information. I would utilize this at my IEP meetings to show parents how a student has been progressing through the year according to what teachers are seeing in the classroom.
I LOVE forms. Yes, this would be a great way to gather data and share with teachers and parents. You'd have to create separate forms for each student though right?
ReplyDeleteWe had the same idea about tweaking it to fit IEP/student need...I thought of it as an additional piece of data gathering, perhaps in addition to the TRAX data we gather?
ReplyDeleteTHis is a GREAT idea! Whether it's for an IEP or just teachers who teach a student from different subject areas who are trying to pinpoint commonalities, times of day, etc. -- anything that might be impacting learning. Thank you for opening up another avenue of thought!
ReplyDeleteI like your use of forms to highlight specific skills. On a similar side note, I've always wanted to start a an ongoing database of student info for 9th grade teachers. Every year it seems like the most struggling students are having problems in multiple classes, but we rarely seem to connect the dots as soon as we probably should to set up effective intervention. It would be nice if we could catch a few more kids before they faceplant, instead of trying to scrape them up after the fact. I've always thought a form/spreadsheet shared amongst teachers could be an effective way to do that.
